My integration of the various theories is to look at unease as the foundation. Unease is desire, wanting what we do not have and aversion, having what we do not want. An emotion is unease in context. Unease in the context of danger is fear. Unease in the context of not reaching a goal as fast as we want is impatience. Unease in the context of repeatedly being denied a reduction in unease is frustration. Unease in the context of loss is sadness. Unease in the context of needing to protect something we desire is anger. Unease in the context of a danger to a relationship is guilt.

Changing the context changes the emotion even if the underlying unease does not change. If I am facing a dangerous task then unease will be high because I have an aversion to putting myself in danger. Unease causes autonomic nervous system changes that include sympathetic activation and parasympathetic withdrawal. If I focus my attention on the danger then I experience fear. If I focus my attention on succeeding at the task then I feel excitement. I can feel both fear and excitement at the same time depending on how much of the context is failure based and how much is success based.

Because emotions are based on unease and changes in unease, and unease both drives and is driven by autonomic nervous system changes, techniques for regulating emotion work much more effectively if we use techniques to shift autonomic function, often increasing parasympathetic activation. Then we need to shift attention to changing the context, otherwise the original state can reassert itself.

For example, if someone cuts me off in traffic I will feel unease (aversion) and my body will respond. If I try to change the context from "that asshole" to "maybe he is in a hurry for a reason" when my body is still activated then changing the context is unlikely to work. If I inhale pause and do a long exhale (activating parasympathetic activity) I am likely to feel my body calm, but if I do not change the context, then I am going to go back to being angry. I need to do the long exhale and then when my body is calm for a few seconds change the context to one that is more helpful. This will shift the emotion from anger "that asshole" to compassion "I hope everything is ok for him". I am then likely to respond by driving safely. Reset, refocus, respond is the process that I teach.

The concept of unease is my interpretation of "desire" and "aversion" from Buddhist psychology. It is tightly connected with the autonomic nervous system and change in unease change the state of the ANS and vice versa. Unease is visceral, not cognitive. The context is provided by memories, imagination, thoughts, perceptions, intentions etc, i.e. the rest of the nervous system.

Mindfulness meditation involves training awareness and attention to recognize the presence or absence of unease and distinguish that from what is helpful ("right speech, right thought, right action). One then learns to place attention on the resources to do what is helpful instead of what will reduce unease in the moment (which is often unhelpful). There is a Zen saying that can be translated, "Do not do what you desire and then you will do what is helpful."

A large part of what I do in therapy is teach people to distinguish reducing unease from doing what is helpful. Then doing what is helpful, and learning to feel pleasure at doing so. Reset, refocus, respond.

As always, an interesting read from you. First, a trivial criticism: the insula and anterior cingulate cortex are cortical structures, not subcortical. True, they are anatomically buried under more superficial regions of cortex, but they are cortical in their structure and in their physical continuity with the rest of the cerebral cortex, which is a large, folded sheet of cells and fibers separated from the rest of the brain by the white matter. Second, it's not just the body that doesn't have language to express itself, most of the brain is also inarticulate. A huge amount of processing takes place outside of our conscious awareness. I imagine it as preverbal. That is where emotions are generated and why words are not always sufficient to describe them.
